Responsibilities
- Provide high-quality preventative care to residents in skilled nursing facilities from a geriatric or internal medicine background
- Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
- Build strong relationships with facilities and maintain a patient-centered approach
- Collaborate with facility staff to ensure seamless integration with existing processes and protocols
- Demonstrate excellent communication, collaboration, and teamwork skills
- Active NP or PA license in the state of practice
- 1 year of experience in geriatric care and documentation review

- Competitive annual salary of $90,000 - $92,000
