Responsibilities:
Provide high-quality preventative care to residents in skilled nursing facilities
Conduct on-site visits and remote work from home, supported by a dedicated scribe team
Build strong relationships with facility staff and maintain a patient-centered approach
Collaborate with other healthcare professionals to ensure seamless patient care
Maintain accurate and timely documentation of patient visits and treatment plans
Requirements:
Active NP or PA license (in the state of practice)
1 year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
Masters-level education in Nursing or a related field
