About the Role:
Carease Health is seeking an experienced Nurse Practitioner (NP) to join our team of dedicated healthcare professionals. As a Nurse Practitioner with our organization, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. With a hybrid work setup that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation, you will have the flexibility to balance your work and personal life.
Responsibilities:
Provide advanced nursing care to residents in skilled nursing facilities, with a focus on chronic disease management and prevention
Conduct patient assessments, develop care plans, and provide medication management and education
Collaborate with facility staff, families, and other healthcare professionals to ensure comprehensive care
Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to proactively manage patient care
Conduct on-site visits and remote work from home, with support from our dedicated scribe team
Maintain accurate and timely documentation, ensuring seamless patient transitions and efficient facility communication
Participate in quality improvement initiatives to enhance patient outcomes and facility performance
Requirements:
Active NP license (in the state of practice)
1 year of experience in Geriatric care and/or Internal Medicine
Excellent communication, collaboration, and teamwork skills
Strong patient-centered approach and ability to build strong relationships with patients and facility staff
