Responsibilities
- Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work
- Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to proactively monitor vital signs and detect abnormalities
- Collaborate with a dedicated scribe team for efficient documentation
- Build strong relationships with the facility and maintain a patient-centered approach
- Be a collaborative team player, working closely with other healthcare professionals to ensure seamless care delivery
- Active NP or PA license (in the state of practice)
- 1 year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
- Hybrid role combining on-site visits and remote work from home
