- Providing high-quality preventative care to residents in skilled nursing facilities
- Conducting on-site visits and remote work from home
- Collaborating with healthcare teams to develop and implement comprehensive care plans
- Utilizing remote patient monitoring, chronic care management, and psychiatric collaborative care to optimize patient outcomes
- Maintaining accurate and timely documentation
- Active NP or PA license in the state of practice
- 1 year of experience in Geriatric care and Documentation review
- Excellent communication, collaboration, and teamwork skills
- Strong knowledge of Geriatric and Internal Medicine principles
