Mobile Health Specialist (Community Paramedic): Job Responsibilities
- Home Visits: Conduct in-person home visits to manage acute illnesses and injuries.
- Treatment & Collaboration: Provide treatment in line with evidence-based guidelines and collaborate with a multi-disciplinary team, including telehealth Emergency Physicians and care coordinators, to handle complex cases.
- Risk Assessment: Identify risk factors, such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation.
- Documentation & Compliance: Maintain patient confidentiality, complete timely electronic records, and adhere to acute care standards.
- Vehicle Requirements: Respond on an on-call basis using a personal vehicle, with the option to use a Care2U vehicle when available and pick up necessary equipment from designated locations.
Compensation and Job Type:
- Full-time: $40 per hour.

Schedule:

Requirements
Requirements:
- Education: Valid NYS Paramedics Certification
- Experience: 3 years of experience in 911, Interfacility or Community Paramedicine
- Certification: BCLS, ACLS, NYS Paramedic. Valid NYS drivers license
Physical Demands:
- Daily use of PPE and electronic documentation.
- Frequent, prolonged standing, walking, and patient/equipment transfers.
- Ability to carry a 30–40 lb backpack during patient visits.
