Here are the main aspects: Mobile Health Specialist (Community Paramedic): Job Responsibilities Home Visits: Conduct in-person home visits to manage acute illnesses and injuries. Treatment & Collaboration: Provide treatment in line with evidence-based guidelines and collaborate with a multi-disciplinary team, including telehealth Emergency Physicians and care coordinators, to handle complex cases. Risk Assessment: Identify risk factors, such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation. Documentation & Compliance: Maintain patient confidentiality, complete timely electronic records, and adhere to acute care standards. Vehicle Requirements: Respond on an on-call basis using a personal vehicle, with the option to use a Care2U vehicle when available and pick up necessary equipment from designated locations.
