Position Summary
The Housing Support Case Manager will provide face-to-face assessments and deliver both pre-tenancy and tenancy support services as part of the Housing Supports Program. This program is designed to assist homeless and housing-insecure participants in obtaining and sustaining stable housing, ultimately improving their overall health and well-being.
The Case Manager ensures participants receive the essential services required to apply for housing, successfully navigate the housing process, and maintain long-term housing stability. This role emphasizes a recovery-oriented approach by using person-first language and person-centered planning while collaborating closely with Managed Care Organizations (MCOs), the Department of Community Affairs (DCA), NJ FamilyCare, and community partners.
Key Responsibilities
1. Collaborate with DCA and NJ FamilyCare to meet program expectations, documenting all contacts in New Jersey’s Homeless Management Information System (HMIS) or an HMIS-comparable system per DMAHS guidance.
2. Conduct comprehensive intake assessments to determine program eligibility and request service authorizations from MCOs.
3. Develop individualized housing support plans with participants using the Housing First model and evidence-based practices such as Motivational Interviewing.
4. Identify, coordinate, and secure resources to assist participants with housing-related expenses.
5. Educate participants on the roles, rights, and responsibilities of both tenants and landlords to foster successful tenancy.
6. Guide participants through housing applications, assist with housing searches, and maintain communication with MCO care management, Housing Specialists, and utilization management staff to ensure timely updates and authorizations.
7. Actively participate in staff meetings, program meetings, and community engagement activities to support continuous program improvement.
8. Communicating with insurance companies to obtain authorizations for payment
9. Uploading and managing documentation in our systems
10. Reviewing documentation for accuracy and completeness
11. Assisting with general office duties and administrative support as needed
Minimum Qualifications
· Bachelor’s degree in social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice from an accredited college/university.
o In lieu of a degree: Minimum of 2 years’ direct experience working with homeless populations required.
· 1–3 years of community health or related experience preferred.
· Familiarity with local housing resources and Medicaid-eligible populations.
· Understanding of cultural and lifestyle diversity issues with the ability to engage diverse populations.
· Strong communication, organizational, and interpersonal skills.
· Experience with Motivational Interviewing and tenant–landlord relationship building strongly preferred.
· Ability to work flexible hours, including evenings/weekends when needed.
· Valid driver’s license, safe driving record, and reliable, insured transportation.
