Clinical Clerk - Home Care Agency

Care Finders Total Care, a Modivcare Personal Care Service, is looking for a Clinical Clerk responsible for ensuring seamless and timely communication between nurses and office staff to ensure proper patient visits are done. You will report to the Branch Manager.

Pay: $15.00-$16.00/Hourly
Schedule: M-F 8:30AM-5:00PM

You will...

• Schedule visits for our field nurses with our clients.
• Update patient information when reported by nurses.
• Organize new admission charts and closed cases for review.
• Prepare weekly reports.
• Prepare patient charts for field nurse visits.
• Ensure scheduled aides are compliant before starting cases.
• Weekly monitoring of aides to ensure home supervisions are done timely.
• Assess authorizations department with audit regarding changes in insurance coverage.
• Liaison between nurses and the coordination department on patient visit interruptions and CHHA compliance.

We are excited to speak to someone with the following…

• High School Diploma or GED
• 1 years healthcare experience preferred
• 2 years of customer service experience preferred
• Experience using HHAeXchange preferred
• Bilingual in English & Spanish required
