Care Coordinator
Full-Time | 32-Hour Work Week
$135,000-$165,000 OTE | Company Car Included
Expansion Markets in NJ - Flexible
Care Connect is expanding.
We are not a call center. We are not a referral mill. And we are definitely not a national franchise routing families through a script.
We operate at the intersection of ethics & performance. Our work feels good. Families never pay us. We do not double dip. We turn down placements that are wrong.
Our team wins by doing the right thing, every single time.
If you are motivated by integrity and results, this is a place where you can build a meaningful career without compromising your standards.
This is fast-moving, high-trust work.
Within weeks, you can help a family move from crisis to stability. You will solve complex care challenges, build real referral relationships, and operate with autonomy and accountability.
We expect performance, clear communication, professionalism, preparation, and follow-through.
In return, we offer flexibility, ownership, growth, and the opportunity to help build something that is shaping the future of ethical senior living placement.
What This Role Actually Is
Not admissions.
It is relationship-driven sales executed with clinical awareness and emotional intelligence.
You will:
• Build and maintain referral-source relationships
• Secure and lead in-services
• Manage 8-15 active family situations at once
• Conduct discovery and assessments
• Coordinate tours and placement strategy
• Drive timelines without pressure or shortcuts
• Own client, referral-source, and market-development follow-through from start to finish
• Maintain accurate and timely CRM documentation, next steps, and pipeline activity
You are expected to run your territory like your business.
That means knowing your calendar, preparing before meetings, owning logistics, following through without repeated reminders, keeping your pipeline organized, communicating early when something is delayed or at risk, and closing loops with clients, referral partners, communities, and teammates.
What Success Looks Like
We believe in clarity.
Success here is measured by both results and execution.
By Day 90, you should be able to demonstrate:
• 3 move-ins or a verified placement pathway demonstrating successful placement competency
• 10 self-sourced leads generated through your own market-development activity
• Repeat referral behavior from key partners
• Meaningful community and referral-source coverage in your market
• A clear, organized pipeline with no loose ends
Just as important is how you get there.
We expect you to:
• Prepare before meetings, in-services, and client interactions
• Own logistics and follow-through from beginning to end
• Communicate early when something is delayed, unclear, or at risk
• Keep your calendar, CRM, pipeline, and market responsibilities organized
• Follow through without repeated reminders
• Take ownership when something goes wrong and correct it quickly
• Represent Care Connect professionally with clients, referral partners, communities, and teammates
You will have autonomy.
You will also be accountable for both your results and how you execute.
What Makes Care Connect Different
We are a specialized senior advisory service with deep expertise in memory care and complex cases.
We do not cut corners to close deals, overpromise families, or operate transactionally.
That's why we have long-term market credibility.
We offer:
• 32-hour full-time schedule
• $135K-$165K OTE
• Company car
• Direct access to leadership
• Clear training and onboarding structure
• Defined 30 / 60 / 90 expectations
• A unique company culture
If you perform, you are trusted.
That trust comes with responsibility.
Who Thrives Here
You likely have experience in:
• Senior living
• Assisted living or memory care sales

• Healthcare relationship management
You understand:
• How referrals are earned
• How to ask for the meeting
• How to follow up consistently
• How to manage multiple cases without losing structure
• How to move families forward without pressure or shortcuts
You are proactive rather than reactive.
You do not need someone reminding you that an event needs food, a partner needs a response, a client needs follow-up, or a commitment needs to be completed.
You can work independently without becoming disconnected from the team.
You know when to solve something yourself and when to communicate early so the team can help.
You carry executive presence in facilities and empathy in living rooms.
Who This Is Not For
• Admissions-only mindset
• Transactional, short-term focus
• Ethical gray areas
• Inconsistent follow-through
• Waiting to be reminded or managed through basic responsibilities
• Disorganization in a fast-moving environment
• Difficulty receiving feedback and adjusting quickly
Families trust us during critical moments.
