What you will do:
- Provide physician orders per agency and regulatory requirements and within the limits of practice. The plan of care and re-evaluation occurs no less often than every 60 days.
- Comprehensive documentation appropriately reflects patient care from admission to discharge utilizing all Oasis assessment tools.
- Serves as a case manager directing multidisciplinary patients' plans of care
- Participates in the agency’s quality assurance program
- Provides written documentation of assessment and care delivery in a professional manner according to policy and within the designated period.
Ideal Candidate:
- Active NYS Registered Nurse Licensure/Registration
- Active NYS Driver’ License, CPR/BLS Card, and COVID vaccinated
- One year of Registered Nurse experience is preferred, however, new graduates are welcome to apply
- Previous homecare experience and/or acute care or rehab experience is a plus
- Oasis Knowledge is a plus
Pay: $78,000.00 - $85,000.00 per year
