Job Description:
Care Connect, LLC is currently seeking a motivated and professional Community Outreach Coordinator to join our growing team.
This is a commission-based opportunity ideal for individuals who enjoy building relationships in the community and want the flexibility to create their own schedule while earning unlimited income.
In this role, you will connect with hospitals, rehabilitation centers, senior centers, assisted living facilities, and other community partners to promote our non-medical home care services and generate client referrals.
Responsibilities:
- Build and maintain relationships with local healthcare and community partners
- Conduct in-person outreach and presentations
- Educate referral sources on available home care services
- Generate and submit qualified client referrals
- Maintain communication with management regarding outreach efforts
Compensation:
- $250 per client who starts services
- Additional commission based on client hours (first 60–90 days)
- Performance incentives available
Qualifications:
- Experience in healthcare, sales, or community outreach preferred
- Strong communication and networking skills
- Self-motivated and dependable
- Reliable transportation required
