What are the responsibilities and job description for the Physical Therapist PRN - Arbor Trace position at Cardon & Associates?
LifeSpan Therapy, a CarDon & Associates partner, is now seeking a PHYSICAL THERAPIST. The Physical Therapist provides compassionate and quality rehabilitation services to residents under the directive of a physician and in collaboration with the Interdisciplinary Care Plan Team. The PT performs therapy or assigns resident to a Physical Therapy Assistant (PTA), communicates sufficient information about the treatment plan to nursing and other personnel to assure a team approach. Documents therapy services and writes progress notes in accordance with Medicare Regulations. When low census, must be willing to travel within a 30 mile radius to provide therapy coverage. (Mileage/time reimbursed). If a therapist is hired as a Float therapist, all Float therapists can be asked to travel to any LifeSpan/Cardon community (travel time reimbursed).

EDUCATION, QUALIFICATIONS, CREDENTIALS: Must be a licensed Therapist who is a graduate of a CAPTE accredited school. Previous experience in a hospital or long term care facility preferred but not required.
