What You'll Do
- The MDS Coordinator plays a critical role in ensuring the accuracy and compliance of resident assessments within the facility.
- This position is responsible for coordinating and completing the Minimum Data Set (MDS) assessments, which are essential for care planning, reimbursement, and regulatory compliance.
- The coordinator collaborates closely with interdisciplinary teams to gather comprehensive clinical data and ensure that resident care plans reflect current needs and conditions.
- By maintaining meticulous documentation and adhering to federal and state guidelines, the MDS Coordinator supports quality care delivery and optimizes facility operations.
- Ultimately, this role contributes to enhancing resident outcomes and sustaining the facility’s reputation for excellence in healthcare services.

- Current and valid Registered Nurse (RN) license in the state of Indiana.
- Experience with MDS assessment processes and knowledge of CMS regulations.
- Strong clinical background in long-term care or skilled nursing facility settings.
- Proficiency in electronic health records and MDS software systems.
- Excellent organizational and communication skills.
