About the Role

Job Responsibilities
- Provide direct patient care, including bathing, dressing, and feeding residents.

- Monitor and document patients' vital signs and health status, reporting any changes to the nursing staff.
- Assist in the development and implementation of individualized care plans.
- Educate residents and their families on health management and wellness practices.
Minimum Qualifications
- Current and valid Licensed Practical Nurse (LPN) license in the state of practice.
- Completion of an accredited practical nursing program.
Preferred Qualifications
- Experience working in a long-term care or skilled nursing facility.
- Basic Life Support (BLS) certification.
