Pupose - The Memory Care Coordinator is responsible for overseeing and supporting residents with memory related conditions in a senior living community
Essential Functions- Job Duties: The following duties are considered essential to the job. Qualified individuals must have the ability, with or without reasonable accommodation, to perform the following duties:
- Work with Residents who require assistance with day-to-day living and overseeing the smooth operations of the department.
- Develops and coordinates memory care programs that promote engagement, safety, and quality of life.
- Assessing resident's needs and helping create individualized care plans.
- Training and supporting staff on dementia care best practices and behavior management techniques.
- Communicating with families about their loved one's condition, care, and progress.
- Monitoring resident well-being and work with caregivers and other healthcare professionals to address change in condition.
- Ensure a safe environment for residents who may wander, become confused, or have behaviorial challenges.
- Maintaining compliance with any federal, state, or local regulations and community policies.
- Establishes and maintains a positive, respectful, and supportive environment for all residents, co-workers, families, and facility guests.
- Assures a welcoming process occurs for all new residents and co-workers.
- Manages Care Aides. Interviews, hires, orients, coaches, counsels, and evaluates Care Aides with guidance from DOW.
- Completes job performance evaluations for Care Aides with guidance of the DOW.
- Performs assessment evaluation for prospective residents with the DOW.
- Works in partnership with the Executive Director, DOW, and Admissions Coordinator to assign residents an appropriate level of care.
- Observes any signs of resident abuse or neglect. Takes immediate action to prevent harm and takes immediate action to prevent harm and report incidents according to Company policy
- Implementation and training in Electronic Health Record system
- May be required to be part of an on-call rotation for nursing department per the discretion of the Executive Director.
- Primary scheduler for the nursing department.
- Performs other duties as assigned
Secondary Functions- Job Duties: The following are secondary to the essential functions listed above:
- Attend and participate in staff meetings, projects, special events, committees, and mandatory training as appropriate
- Maintains safety and health rules by participating in training along with implementing and following protocols related to OSHA & CDC regulations and Cardinal Senior Management Standards as required
- Promotes and encourages individualized resident independence while maintaining resident dignity, individuality, confidentiality and following infection control policies.
- Works as a member of a team to provide essential nursing services and uses effective communication skills.
Education & Knowledge Requirements- Specific education, certification, and licensure:
- High School Diploma or equivalent
- Experience with Microsoft Office
Essential Job-Related Experience/Abilities:
- 1-2 years experience working in a senior living community preferred
- 1-2 years experience working as a care giver, proving care and/or passing medications preferred
- Ability to read, write, speak and understand the English language
- Ability to walk continuously, bend frequently, and sit occasionally
- Ability to complete and maintain documentation regarding a resident’s status
- Ability to operate computer systems and phone systems.
