Join our dedicated team as a Case Planner in the Prevention and Special Medical Program, where you'll work directly with families at risk of foster care placement due to substance use or mental health disorders.Responsibilities:
- Guide families through a structured, three-phase treatment model: Initial, Baseline, and Stabilization.
- Conduct comprehensive assessments and develop individualized service plans.
- Coordinate multidisciplinary interventions tailored to each family's unique needs.
- Build meaningful relationships with families over an average case duration of 12 months.
- Collaborate with internal and external providers to ensure safety, stability, and well-being.Requirements:
- BSW or bachelor's degree in human services.
- Some knowledge of the child welfare system.Additional Requirements:
- Fluency in English and Spanish a Plus
