Registered Nurse - Nurse Care Manager
At CMCS (Cardinal McCloskey Community Services), our mission is to protect, empower and promote independence for at-risk children and families and those with developmental disabilities through quality community-based services. A COA Accredited Agency, CMCS has provided services in locations throughout Westchester and Rockland Counties, as well as the Bronx and Manhattan since 1946.
As the Registered Nurse - Nurse Care Manager is responsible for the well-being of individuals with disabilities who live in our group homes and/or attend our day habilitation programs. The RN provides care management, medical consultation and triage services, and medical education to the individuals receiving services. The Nurse Care Manager is also responsible for ensuring program team members receive appropriate medical training and support, ensuring they are equipped to provide medical support in the absence of the nurse.
The Registered Nurse - Nurse Care Manager will ensure that individuals are receiving routine care and will conduct weekly site-visits to review/update medical records, check medication for refilling and expiration and complete any necessary follow-up. The RN is responsible for developing and updating Plans of Nursing Services (PONS) annually and as needed and ensures staff receive appropriate training. Additionally, he/she is responsible for monitoring all aspects of medication administration, including but not limited to- reviewing/updating individual Medication Administration Records (MARs), investigating and issuing/correcting medication errors, reviewing medications for compliance with OPWDD standards, disposing of and discontinuing medication and conducting self-administration assessments with individuals receiving services.
Requirements:
· Registered Nurse with current New York State Licensure
· Previous experience providing health care management and/or nursing to individuals with developmental disabilities and/or psychiatric disabilities preferred
· Previous experience working with a medically fragile population preferred
· Must have daily access to an automobile and be willing to travel between locations throughout the Bronx and Westchester County
