GENERAL SUMMARY

Provides direct care and coordination of medical services to program participants.

 

ESSENTIAL JOB FUNCTIONS*

  • Provides direct care, as needed.  
  • Provides training in relevant health topics for staff and clients, as needed.
  • Coordinates medical services with each participant’s primary physician or medical clinic.
  • Oversees all health care services provided to program participants.
  • Participates in the ongoing interdisciplinary evaluation and review of each participant’s needs and status.
  • Obtains reports and approval of treatment plans from primary physicians.
  • Documents recommendation in participants’ records resulting from periodic reviews.
  • Advises program management and staff of any medical problems that may hinder a participant’s progress or that may be negatively impacted by program activities.
  • Completes, signs and/or reviews monthly progress notes in accordance with regulations established by the Commonwealth of Massachusetts-Executive Office of Health and Human Services (EOHHS).
  • Serves as mandated reporter for suspected cases of abuse and/or neglect to the Disabled Persons Protection Commission (DPPC).

PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

  • Registered Nurse (RN) licensed by the Massachusetts Board of Registration in Nursing.
  • Minimum 1 year prior experience in a community health setting.
  • Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently.
  • Valid driver’s license in the state of Massachusetts required.
  • Successful completion of all state and Agency mandated trainings required upon hire.
  • Must be compassionate and patient.

 

SUPERVISORY RESPONSIBILITY

WORKING CONDITIONS/PHYSICAL DEMANDS

  • Frequent standing, walking, bending, and twisting.
  • Ability to lift up to 50 pounds.
  • Ability to pull up or transfer individuals weighing up to 250 pounds.
