POSITION PURPOSE:
Provides specialized wound care to residents in an assisted living setting, ensuring the highest standards of treatment, prevention, and healing. Assists in coordinating clinical care related to skin integrity and wound management, while supporting the nursing team and carrying out responsibilities delegated by the Director of Nursing.
ESSENTIAL FUNCTIONS OF POSITION:
• Provides direct wound care to residents, including assessment, treatment, dressing changes, and ongoing monitoring of skin integrity.
• Develops, implements, and updates individualized wound care plans in collaboration with physicians and the interdisciplinary team.
• Monitors residents for risk of skin breakdown and implements preventative measures to maintain skin health.
• Documents wound assessments, treatments, and progress accurately and in a timely manner.
• Communicates regularly with physicians, families, and care teams regarding wound status and changes in condition.
• Ensures physician orders related to wound care are reviewed, followed, and updated as needed.
• Provides education and training to nursing staff on proper wound care techniques, infection control, and prevention strategies.
• Assists in ensuring compliance with company policies, procedures, and regulatory standards related to wound care and infection prevention.
• Participates in quality improvement initiatives related to wound care outcomes and infection control.
• Collaborates with nursing leadership to address clinical concerns and improve resident care practices.
• Assists with audits, tracking, and reporting of wound care data and outcomes.
• Participates in interdisciplinary meetings, care conferences, and staff meetings as needed.
• Supports overall nursing operations and provides direct care assistance when necessary.
• Performs additional duties as assigned.

Education/requirements:

Two years previous nursing experience required,

Preferably working with a geriatric long-term care population; or equivalent combination of education and experience.

Licensed to practice nursing in state of employment.

Qualifications:Education/requirements:
