Role Description The Nurse Navigator is a full-time, on-site role located in Columbia City, IN, focused on guiding patients through the healthcare system and coordinating care across services. Day-to-day responsibilities include assessing patient needs, providing education on diagnoses and treatment plans, and serving as a primary point of contact for patients and families. The Nurse Navigator collaborates with physicians, nurses, and other care team members to schedule appointments, facilitate referrals, and ensure timely follow-up and continuity of care. This role also involves monitoring patient progress, addressing barriers to care such as transportation or insurance issues, documenting interactions in the medical record, and participating in quality and safety initiatives to support optimal outcomes.
