Role Overview:
The Registered Nurse (RN) provides skilled home health nursing care to patients in their residences. This role includes comprehensive patient assessment, development and management of Plans of Care (POC), OASIS assessments, coordination with interdisciplinary teams, and patient/family education. The RN also ensures compliance with payer requirements and supervises HHAs/LVNs as applicable.
Position: Registered Nurse (Home Health)
Hours: 40 hrs/week (8 hrs/day)
Location: Deland, FL 2720
Pay: $1900 to $2300
Key Responsibilities:
- Perform comprehensive home health and OASIS assessments
- Develop, implement, and evaluate Plans of Care (POC)
- Deliver skilled nursing care using OSHA-compliant infection control practices
- Maintain accurate, timely clinical documentation
- Educate patients and families on medications, treatments, safety, and self-care
- Assess and manage pain using appropriate interventions
- Supervise HHAs/STNAs/LVNs
- Coordinate referrals and interdisciplinary care
- Ensure compliance with third-party payer requirements
- Participate in team meetings and on-call coverage as assigned
Licensure & Certifications:
- Active Florida RN license
- Current CPR certification
- Valid driver’s license
- Reliable transportation with insurance
Experience:
- 1–2 years RN experience in a clinical setting
- 2 years home health experience
- OASIS experience (required)
- Homecare Homebase (HCHB) software experience (required)
Skills:
- Strong assessment and critical-thinking skills
- IV therapy, venipuncture, wound care, catheter care
- Medication administration and patient education
- Ability to work independently and manage caseloads
- Excellent communication and documentation skills
