We are seeking a compassionate and experienced Licensed Practical Nurse (LPN/LVN) for a Home Health travel assignment. The ideal candidate will have prior home health experience, strong clinical judgment, and the ability to work independently while delivering high-quality patient care in home settings.
Job Details

- Location: Pittsfield, Massachusetts
- Shift: Days | 8:00 AM – 4:30 PM
- Hours: 40 hours per week (8-hour shifts)
- Start Date: August 17, 2026
- Duration: 13 Weeks
Requirements
- Active Massachusetts LPN License (must be in hand before submission)
- Current BLS Certification (AHA or American Red Cross)
- Minimum 1 year of acute nursing experience (hospital experience preferred)
- Home Health experience required
- Experience with OASIS documentation highly preferred
- Infusion therapy experience preferred
- Valid driver's license, auto insurance, and a reliable personal vehicle (required)
- Knowledge of home health regulations and reimbursement guidelines
- Strong communication, documentation, and time management skills
- Ability to work independently with minimal supervision
- Experience managing and self-scheduling a patient caseload is preferred
Responsibilities
- Provide skilled nursing care to patients in their homes.
- Assess, monitor, and document patient conditions accurately.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed.
- Educate patients and families on disease management, medications, and home care.
- Complete accurate clinical documentation, including OASIS assessments when applicable.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Maintain patient confidentiality and comply with all regulatory standards.
- Demonstrate sound clinical judgment, critical thinking, and problem-solving skills.
- Travel between patient homes to provide scheduled visits while ensuring high-quality, compassionate care.
Pay: $2,000.00 - $2,300.00 per week
