LVN / LPN – Case Manager
Location: CA
Schedule: Day Shift | 5×8-Hour (08:00–16:30)
Contract Length: 8 Weeks (Opportunity to Extend)
Job Summary
We are seeking an experienced LVN/LPN Case Manager to support discharge planning and care coordination within an acute care hospital setting. This is a clinical, non-RN role focused on managing high-volume discharges, coordinating post-acute services, and ensuring safe, timely patient transitions. Candidates must have direct adult acute care hospital case management experience.
This position plays a vital role in interdisciplinary collaboration and patient flow, particularly during high-volume weekday and weekend discharges.
Key Responsibilities
- Coordinate and manage patient discharge planning from admission through transition of care
- Conduct comprehensive needs assessments and develop individualized plans of care
- Coordinate DME, transportation, home health, SNF placement, and other post-acute services
- Review admission criteria and determine medical necessity using evidence-based guidelines
- Initiate and follow up on prior authorizations
- Collaborate with physicians, nursing staff, social services, physical therapy, and ancillary departments
- Maintain compliance with CMS, DRG, HIPAA, Joint Commission, and Core Measures
- Support utilization review, appeals, and denial management as assigned
- Manage a case workload of up to 50 discharges, with higher volumes on weekends
- Document accurately and timely using Epic
Required Qualifications – HARD REQUIREMENT
Adult Acute Care Hospital Case Management Experience is Mandatory
- LVN/LPN: Minimum 2 years of case management experience in an adult acute care hospital
- Non-LVN candidates: Must have 10 years of adult acute care hospital case management experience
- Behavioral Health, Outpatient, or Group Term Insurance experience does NOT qualify
- Epic charting experience required
Preferred Skills & Experience
- Strong discharge planning and care coordination background
- Familiarity with ICU, Med-Surg, SDU/PCU/IMC/Observation, and Surgical Services workflows
- Experience with worker’s compensation case management
- Excellent communication, prioritization, and problem-solving skills
