Job Summary
Job Position: Licensed Clinical Social Worker
Location: Centralia, IL
Shift: 5x8, days
Duration: 13 Weeks
Guaranteed hours: 40
Responsibilities
- Conduct thorough clinical assessments and diagnostic evaluations to determine patient needs and develop individualized treatment plans.
- Provide individual, group, and family therapy utilizing evidence-based practices such as cognitive-behavioral therapy, psychodynamic therapy, behavioral therapy, and applied behavior analysis.
- Deliver specialized care for patients with developmental disabilities, autism spectrum disorder, PTSD, addiction issues, Alzheimer's disease, geriatrics, and other behavioral health conditions.
- Facilitate crisis intervention and crisis management to ensure patient safety and stabilize acute mental health episodes.
- Collaborate with interdisciplinary teams to coordinate patient care across hospital units, outpatient clinics, home health services, and community organizations.
- Maintain accurate clinical documentation in compliance with HIPAA regulations and medical documentation standards using Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems.
- Engage in case management activities including patient assessment, discharge planning, and follow-up care to promote long-term recovery.
- Participate in clinical research initiatives related to mental health treatment outcomes and behavioral health interventions.
- Ensure adherence to confidentiality policies while managing sensitive medical records and social work documentation.
- Support patients through intake processes, treatment planning sessions, progress monitoring, and discharge procedures.
Requirements:
- IL LCSW must be in hand
- Outpatient Behavioral Health Clinic LCSW
- 1 year of experience required
- Certifications: LCPC, LCSW, or LMFT
- Epic experience required
Pay: $2,650.48 - $2,820.57 per week
