Patient Care Coordinator (Per Diem) – Hybrid | Montclair, CA
Must have an active LVN license
We’re hiring a Per Diem Patient Care Coordinator to support our hospice team. This role is hybrid with some in-office and remote days. Weekend holiday availability required (those shifts are remote). Weekday office hours: Mon–Fri, 8:30 AM–5 PM.
Key Responsibilities
- Support patients/families with calls, scheduling, and medication refill requests
- Manage schedules for LVNs, RNs, CHHAs, and on-call staff
- Coordinate PRN visits with clinical team
- Update patient info in EMR
- Provide strong customer service and resolve patient inquiries
- Partner with other departments (PT, wound care, etc.)
- Order/track DME supplies
- Complete prior auths, verify benefits, and prep charts for billing
Requirements
- Active LVN license (CA)
- 1 year in patient care coordination (hospice preferred)
- Bilingual a plus
- Strong computer skills (MS Office, HospiceMD)
- Organized, flexible, and reliable
Physical Requirements
- Prolonged sitting/computer work
- Periodic standing/moving
- Able to lift 20 lbs

Pay: $32.00 - $36.50 per hour
