Duties and Responsibilities:
Provide direct outpatient care for an assigned patient population.
Perform nutrition care process including assessment, diagnosis, intervention, monitoring and evaluation.
Able to work in a fast-paced, patients first environment, and to organize, prioritize and problem-solve effectively
Use nutrition diagnosis to describe nutrition problems so that they are clear to the patient and each member of the patient’s interdisciplinary team.
Able to identify and perform brief interventions that improve patients’ nutrition and health outcomes.
Provide leadership and direction related to nutrition care with interdisciplinary team members including physicians, nurses, residents, students, and other allied health professionals.
Assist with the creation and revision of educational material related to patient care.
Actively contribute to department and corporate initiatives, including committees and projects.
Participate in targeted professional growth activities and continuous personal development.
Utilize clinical software applications to facilitate care including AthenaHealth and Google Drive
Help design and lead the corporate wellness plan
Assist with CenteringDiabetes and CenteringPregnancy classes
Assist with community initiatives that help to improve the health of the service population of CMC
Other duties as assigned.
Qualifications:
Maintain a state license by the Alabama Dietetic Licensure Board
Minimum of two years of experience in patient care following board licensure is desired
Submits to the necessary components to maintain privileging within the clinic including background checks, random drug screens, peer reviews by CMO, certification of fitness to perform duties, and willingness to comply with the various quality improvement programs
Be self-directed and independent, but also work well in an interdisciplinary team setting.
Demonstrate a high degree of competency with computers, including the ability to use an Electronic Medical Record reporting system (training will be provided).
Possess excellent oral and written communication skills, with patients and colleagues, that augment positive patient outcomes.
Must be passionate about healthcare and committed to our mandate of putting “Patients First”.
