The RN Case Manager will be responsible for assessing patient needs, developing individualized care plans, coordinating hospice care services, and providing emotional support throughout the end-of-life journey.
Key responsibilities include:
- Clinical Assessment and Care Planning
- Symptom Management and Pain Control
- Emotional Support and Counseling
- Care Coordination and Collaboration
- Documentation and Compliance
- On-call Rotation for after-hours and weekends
- Registered Nurse (RN) licensure for the state of Ohio
- Clean driving record, and valid driver’s license and auto insurance
