Licensed Vocational Nurse (Home Health)
Location: Canoga Park, CA
Schedule: Part Time
Home Health Agency is seeking a compassionate and reliable Licensed Vocational Nurse (LVN) for a part-time position serving a patient in Canoga Park, CA. Preferably Spanish speaking per family request.
Responsibilities
· Provide skilled nursing care in the patient’s home per the plan of care, medication administration, and treatments
· Accurately document care in the EMR and communicate patient status changes to the clinical team
· Collaborate with, RN Supervisors, DPCS, and families to ensure continuity of care
Follow all agency policies and California home health regulations
Qualifications
· Active California LVN license in good standing
· Minimum 1 year of clinical experience
· Current BLS/CPR certification
· Reliable transportation and valid driver’s license and auto insurance
· Current Physical and TB/ CXR
· Strong communication, organization, and documentation skills
