Hammond Clinic 7905 Calumet Ave Munster, Indiana 46321 The Community Health Improvement Coordinator manages and implements the community benefit function for the assigned hospitals of Franciscan Alliance. This position serves as the hospital or regional leader that oversees the process, relationships, data, and strategies to meet health needs in the communities the hospital serves. This includes direct responsibility for meeting the IRS requirements necessary to remain a not-for-profit hospital penalty free and alignment with system-wide strategic direction. The coworker will collect and assess relevant data; develop, implement, and evaluate community interventions; engage stakeholders in decision making; and manage cross sector internal and external partnerships. WHAT YOU CAN EXPECT Schedule: Monday - Friday, 8am - 4:30pm Some travel throughout Lake County, but mostly between Hammond and Crown Point locations. Monitor Community Health trends, population health trends, and activities of appropriate organizations such as the Association for Community Health Improvement (ACHI). Attend appropriate conferences and meetings as the hospitals representative, and shares findings with leadership as necessary. Coordinate creation of community benefit reports and ensures accurate reporting for the state government, federal government, and local community benefit reporting Facilitate training of the Community Benefit Inventory for Social Accountability (CBISA) Plus Online Tracking System and the linked paper CBISA reporting of activities and its elements to Boards, leadership meetings and employees for all hospitals of Franciscan Health Central Indiana area Manage the Community Health Needs Assessments (CHNA), and manage the related goal setting for interventions, educational initiatives, and social determinant related activities. Support community initiatives to promote collaborative arrangements that meet the health needs of the community Work with the Franciscan Alliance Accountable Care Organization (ACO)s, Foundation, and Mission leaders in providing support in epidemiology, community benefit knowledge and tracking, and to develop mutually beneficial goals. Identify opportunities for program development and recommend program goals, including working across departments and Franciscan entities to identify and implement cross-cutting strategies and activities. Collaborate with senior and departmental leaders to set priorities and develop strategies for advancing community benefit initiatives throughout the system in accordance with system and federal requirements.
