- Support clients in achieving independence and life skills development
- Develop and implement individualized care plans with families and healthcare professionals
- Monitor client health and document daily progress and observations
- Maintain a clean, safe, and supportive environment
- Follow all health, safety, and company regulations
- Participate in training and continuous professional development
- High school diploma or equivalent
- Experience in caregiving, home care, or working with individuals with disabilities
- Strong communication and interpersonal skills
- Compassionate and patient-centered approach
- Ability to manage time effectively and stay organized
- Ability to work independently and within a team
- DSP certification or similar healthcare credential
- Experience with electronic health records (EHR systems)
- Bilingual skills (highly preferred)
