Duties & Responsibilities
Personal Care & ADLs
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, eating, and hygiene
- Support mobility, transfers, and positioning, including use of equipment such as Hoyer lifts when required
- Provide continence care and ensure residents’ comfort and dignity
- Encourage independence while providing respectful, person-centered care
Health-Related Support
- Assist with medication administration or reminders per care plan and RN delegation
- Monitor and document vital signs, symptoms, and changes in condition
- Observe physical and mental health status and report concerns promptly to nursing or leadership
- Support residents with chronic conditions (e.g., diabetes, CHF, COPD, chronic pain)
- Follow HIPAA guidelines and maintain confidentiality at all times
Behavioral & Mental Health Support
- Provide support for residents with anxiety, depression, cognitive impairment, or behavioral needs
- Use appropriate techniques such as redirection, reassurance, and de-escalation
- Respond to behaviors including verbal aggression, agitation, or repetitive actions
- Promote a calm, structured, and supportive environment
Nutrition & Meal Support
- Prepare and serve nutritious meals based on dietary needs and preferences
- Assist residents with eating as needed
- Support meal planning and ensure proper food handling and safety
Household & Environmental Support
- Perform light housekeeping, including cleaning common areas and resident spaces
- Assist with laundry and maintaining an organized living environment
- Follow infection control procedures and maintain cleanliness standards
Documentation & Communication
- Accurately document care, services, and observations each shift
- Report incidents, behavior changes, and health concerns in a timely manner
- Communicate effectively with team members, nurses, and supervisors
Safety & Supervision
- Ensure resident safety at all times, including during transfers and daily activities
- Follow all care plans, facility policies, and emergency procedures
- Monitor for fall risks, skin concerns, and environmental hazards
- Use proper body mechanics and safety protocols to prevent injury
Skills
- Proven experience working in group homes, assisted living, nursing homes, or home health care environments
- Knowledge of patient care techniques, including assisting with ADLs, medication management, and patient monitoring
- Ability to work effectively with individuals with developmental disabilities or disabilities requiring specialized support
- Strong observational skills for patient safety and health status changes; ability to document accurately while respecting HIPAA guidelines
- Physical stamina for heavy lifting, assisting with transfers using Hoyer lifts or other equipment, and long periods of standing or walking
- Compassionate communication skills to build trust and rapport with clients and their families

Qualifications
- High school diploma or GED (required)
- Experience in assisted living, home care, or healthcare preferred
- Ability to work with vulnerable adults and individuals with mental health needs
- Strong communication and interpersonal skills

Pay & Benefits
- Starting Pay: $17.00 per hour
- Wage Increase Structure:
- Increase to $18.00/hour after 6 months
- $0.35 raise every 6 months thereafter based on performance
