Our Quest Tailored Day Services Program:

• In this program, we serve adults with developmental disabilities in their homes or the community, focusing on post-secondary education, vocational interests, and other personal goals. Community Mentors travel to meet 1-3 individuals per day and provide one-on-one direct support
  
  

As a Community Mentor you will be responsible for:

• Provide individual support in the home or community setting as identified in the participant’s Individualized Personal Plan
• Work closely with the participant’s support team to identify needs and develop plans
• Navigate private and public resources to create opportunities for growth, develop skills, and meet specific goals outlined in the Individual Service Design and IPP
• Travel with participants throughout the local community to attend social events, recreational activities, school, etc
• Advocate for participants’ rights and individual choice in all settings and provide services in accordance with HCBS Final Rule guidelines
• Complete required documentation, including daily notes, activities using Buildability digital platforms, and written reports of client progress or barriers encountered in meeting goals
• Perform other duties as assigned to assure the efficiency of program services
  
  

Schedule:

• Part-time position (15 - 25 hours per week)
• Day shift, Monday to Friday
• Weekend availability
• Hours assigned are subject to change weekly based on availability
  
  

Work location:

• San Fernando Valley, East Valley, Burbank, Glendale, North Hills
  
  

Compensation:

• Starting at $19.00 - $22.00/hour (depending on prior experience and qualifications)

Required Qualifications:

• Strong interpersonal skills and the ability to interact with individuals with disabilities, their family members, employers, and others in a positive and constructive manner
• High school diploma or GED
• Must pass a DOJ Live Scan fingerprinting background clearance
• Valid California Driver’s License, automobile insurance, and reliable transportation
• Willing to travel: up to 75%
• Cultural awareness and adaptability
  
  

Preferred Qualifications:

• Fluent in Spanish
• Familiarity with the Los Angeles Metro system (MetroGo app, route planning, etc.)
• Experience with resource navigation (Transportation resources, accessibility resources, etc.)
• Crisis management skills
• Prior experience supporting individuals with disabilities. If you don’t have experience, we will provide on-the-job training. Some of our best staff did not have experience prior to being hired at BuildAbility!
