What are the responsibilities and job description for the Clinical Pharmacist position at Buffalo Medical Group?

We are currently seeking a Full Time Clinical Pharmacist. This position will work throughout all of locations in Williamsville, Lockport, Orchard Park and Downtown Buffalo. .
Schedule:
Full Time - Monday - Friday 8:00am - 4:30pm
Job duties included, but are not limited to:
- Works collaboratively with providers to provide diabetes education.
- Works collaboratively with providers to provide medication therapy management.
- Provide assistance with medication reconciliation.
- Provides any necessary drug information as requested.
- Scheduling of any needed appointments for patients.
Qualifications:
- Graduate of an ACPE Accredited Pharmacy School.
- Holds or is eligible for NYS licensure (with Immunization rider).
- Completion of a PGY1 or PGY2 (preferred) Residency Training Program or 5 years experience in a relevant practice setting.

Pay: $98,446.40 - $147,669.60 per year

License/Certification:
- PGY-2 in ambulatory care (Preferred)
- PGY-1 residency (Preferred)
