Responsibilities
· Lead System Integration Facilitation (case conferencing) for high acuity clients in the homeless service system (Housing Link). Bring together non-profit service providers, government services, housing providers, and the client to create service plans that promote long-term housing stability.
· Oversight of the street outreach provider network to ensure all the necessary tasks of street outreach are occurring in a coordinated fashion. This includes verification of homelessness, canvassing, case management, information and referral, and diversion.
· Provide assistance with data quality for the coordinated entry system and use data to provide insights for non-profits on improving performance.
· Work directly with clients, usually in conjunction with assigned case manager, to assist in resolving housing barriers.
Qualifications:
· Associates or Bachelor’s Degree in Human Services related field, Social Work, or other related field. If no higher education, equivalent experience in case management related roles will be considered.
· Direct case management experience in the field of Human Services required. Direct experience working with clients experiencing housing instability preferred.
· Knowledge of and experience in homelessness, housing issues, behavioral health, and Medicaid population preferred, particularly in working with individuals with a serious mental illness and/or co-occurring disorders.
