Job Summary:
Supervises and coordinates all in-center and home care Personal Care Aides (PCA) to ensure participant care activities are aligned with their plan of care.
Schedule:
First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Duties/Responsibilities:
- Ensures center and home care PCA operations and environment are within audit standards, including completion of all required logs.
- Supervision of in-center and home care PCAs.
- Will attend and participate in all IDT Meetings.
- Responsible for the master schedule, center, and home care PCA schedules.
- Responsible for ensuring Participants care planned interventions are being completed as pertained to their scope of care.
- Ensures that participants are receiving appropriate supplies per their needs by being involved in the coordination of the delivery of supplies with appropriate departments.
- Ordering appropriate durable medical equipment and completing authorizations as needed.
- Complete necessary documentation and encounters in the EMR in a timely manner.
- Review documentation and encounters in the EMR in a timely manner.
- Responsible for completion of audits, including but not limited to home care PCA documentation
- Responsible for completing initial and annual competencies for the Direct Care Aide Scheduler, all in-center and home care PCAs.
- Responsible for reviewing, verifying, and approving electronic timesheets for the Direct Care Aide Scheduler and all PCA payroll, mileage, and expense reimbursements.
- Completes all initial, six months and significant change assessments in the home to assess for participants resources, safety, functionality and living environment.
- Assist with developing a plan of care based on the in center or home care need and which incorporates the plans of other disciplines and continuing care needs; includes the participant/family in developing or revising the plan of care; makes referrals to multidisciplinary support services.
- Participate in, and complete on-going training as directed.
- Monthly supervisory visits with PCAs.
- Participate in on-going Quality Initiative training as directed.
- Other duties as assigned.
Supervisory Responsibilities:
- 8 direct reports depending on census

Education and Experience:
- Bachelor's degree
- 5 years' experience in:
- Supervision, scheduling, Pacelogic, and Prime Suite
- Microsoft Office (intermediate to advanced)

Requires valid State drivers license with a clean driving record.

Successful completion of background check, physical, drug screening, TB Screening
Physical Requirements:
- Must be able to move intermittently throughout the workday.
- Must be able to lift, push, pull, and move a minimum of 25 pounds.
- Must be able to assist with the evacuation of Participants.
