What are the responsibilities and job description for the Registered Nurse position at Buckeye Hospice and Palliative Care Services?
As a Case Manager (RN) for Buckeye Hospice at our McConnelsville/Zanesville and surrounding locations you will be responsible for the assessment and care of Hospice patients in nursing facilities and home settings.
TYPICAL SCHEDULE IS FULL-TIME. 8a-4:30p Mon-Fri w/ on-call rotation for week nights and weekends.
Job Responsibilities
- Provides the initial assessment and comprehensive nursing assessment of patient needs.
- Implements the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions.
- Communicates the patient’s progress with the patient/caregiver and other disciplines as well as directs, supervises and instructs direct care staff members and nonprofessional hospice aide staff to deliver quality care and coordinates delivery of care services.
- Assessment and care of Hospice patients in nursing facilities and home settings.
Job Requirements
- RN License is REQUIRED.
- Valid Drivers License and reliable transportation that is insured in accordance with Ohio state requirements.
- Minimum of 1 to 4 years as a RN working with hospice services or home health agency.
- On-Call Rotation.
