Role Description The Hospice Liaison is a full-time, on-site role based in Medina, OH, responsible for building and maintaining relationships with healthcare providers, facilities, and community partners to promote Buckeye Hospice and Palliative Care Services. This role includes educating referral sources and families about hospice and palliative care options, eligibility criteria, and the benefits of services offered. The Hospice Liaison will coordinate with clinical and administrative teams to support smooth patient referrals, timely admissions, and positive care transitions. Daily activities involve outreach visits, presentations, and follow-up communication to ensure referring partners and families receive clear, accurate information. The role also includes tracking outreach efforts and referral trends to support organizational growth while upholding the company’s mission of compassionate, dignified care.
Qualifications
- Demonstrated knowledge and experience in hospice care, end-of-life care, and palliative care, with a strong commitment to patient dignity and comfort.
- Background in home care or related community-based healthcare services, with an understanding of care coordination and family support needs.
- Sales or liaison experience in healthcare, including relationship-building with referral sources, conducting outreach, and meeting growth objectives.
- Strong communication, presentation, and interpersonal skills, with the ability to explain complex care options in a clear, compassionate manner.
- Ability to work collaboratively with clinical and administrative teams, manage multiple priorities, and maintain accurate records of outreach and referrals.
- Prior experience in hospice, home health, or palliative care settings preferred.
- Valid driver’s license and reliable transportation for on-site visits within the Youngstown and surrounding service areas.
- Associate or Bachelor’s degree in a healthcare, social services, or related field preferred; equivalent experience considered.
