Job Summary:
We are seeking a Registered Nurse (RN) to join our team as a Case Manager at Buckeye Hospice and Palliative Care Services in our Zanesville, Ohio market. The ideal candidate will have a passion for delivering patient-centered care and be committed to making a difference in the lives of those we serve.
Responsibilities:
- Provide initial and comprehensive assessments of patient needs, developing personalized care plans to address their health and social needs.
- Implement and revise care plans as needed, ensuring that patients receive the best possible care and services.
- Communicate effectively with patients, families, and healthcare teams to coordinate care delivery and ensure seamless transitions.
- Supervise and instruct direct care staff members and non-professional hospice aide staff to deliver quality care.
- Participate in on-call rotation, providing 24/7 coverage for patients and families in need.
Requirements:
- RN License is REQUIRED.
- Valid Driver's License and reliable transportation that is insured in accordance with Ohio state requirements.
- Minimum of 1-4 years of experience as a Registered Nurse working with hospice services or home health agency.
- On-Call Rotation.
Work Environment:
- Multiple locations in Zanesville and surrounding areas.
- Long-term care settings, including nursing facilities and home settings.
- Monday to Friday, with occasional on-call and weekend availability.
Salary:
$78,000 - $84,000 annually
