An Average Day:
(Includes, but not limited to)
- Provide education and technical support to the program in developing, implementing and maintaining quality improvement activities
- Collaborate with the QAPI Chairperson performance improvement taskforces and support the Executive Director (chairperson of the QAPI Committee)
- Collaborate with leadership, staff, vendors, and community in processes that continually improve the quality in all levels of care delivery
- Audit, monitor, track and trend high-risk, high-volume problem-prone areas of hospice in
- accordance with the QAPI plan
- Support agencies in review of patients Not Taken For Care (NTFC) and live discharges
- Collaborate with leadership and staff to improve rate of eligible admissions
- Provide education regarding documentation towards LCD guidelines, decline and eligibility
- Proactive identification of issues and tracking of corrective action to minimize negative impact and maximize learning opportunities
- Collaborate with leadership in implementing regional/agency/organizational goals and objectives
- Develop and maintain relationships and effective communication with all levels of physicians and staff to facilitate problem identification and resolution
- Effectively communicate, both verbally and in writing, to assure that staff, leadership and QAPI Committee is informed of the status and issues related to the Performance Improvement Projects (PIP), utilization management and risk management
- Work closely with location, regional and corporate leadership to address patient care issues with a goal toward resolution while maintaining confidentiality
- Oversee QAPI processes and prepare reports for the QAPI Committee
- Complete annual program evaluation for submission to the QAPI Committee, Governing Body and Regional VPO and VPCO
- Audit medical records in collaboration with location QAPI Committee to ensure integrity of the medical records is sustained
- Provide direction and coordination of quality improvement data collection and aggregate activity utilizing continuous quality improvement principles and methodologies according to the QAPI
- Support agency activities related to IDT clinical record review and SNF chart audits for data collection/aggregation and reporting the location
- Compile, aggregate and assist with trending of quality data in the following areas: patient care, safety, risk management, chart/documentation audits, infection control, outcomes, public reporting, hospice compare, and customer satisfaction
- Participate in continual survey and federal audit readiness
- Collaborate with the Informatics Nurse to manage and maintain accurate, reliable data that complies with regulations, accreditation, and agency standards
- Adhere to all state, federal and organizational timelines related to entry and submission of quality measures, including HIS.
- Plan, organize and facilitate the gathering and organization of HIS data from assigned locations including creation and verification of all HISA and HISD documents.
- Collaborate with location leadership to trend and track HIS data, provide staff education as directed, and ensure continuous quality improvement for the Hospice Quality Reporting Program
- Other duties as assigned by Regional VPCO/RVPO
Requirements:
- Must have an RN licensure, BSN is strongly preferred
- Must have a minimum (4) years of experience as a nurse, hospice nursing strongly preferred
- Preferred minimum of one (1) year experience in a leadership role
