Duties
- Conduct comprehensive evaluations of patients’ physical conditions, including mobility, strength, balance, and functional abilities.
- Develop tailored care plans that incorporate evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, neurology, pediatrics, geriatrics, and inpatient rehabilitation strategies.
- Deliver hands-on therapy sessions focused on patient rehabilitation, including exercises for musculoskeletal injuries, neurological conditions, or post-surgical recovery.
- Collaborate effectively with healthcare professionals such as physicians, nurses, occupational therapists, and social workers to coordinate care plans and discharge planning.
- Manage clinical documentation standards by accurately recording treatment progress, updating care plans in electronic health records (EHR) systems or electronic medical records (EMR), and ensuring compliance with healthcare regulations.
- Educate patients and their families on home exercise programs, safety measures, and community resources to support ongoing recovery.
- Perform discharge planning activities to ensure seamless transition of care from hospital or outpatient settings to home environments.
Experience
- Prior experience practicing in a physical therapy role within home health settings is highly preferred.
- Demonstrated outpatient clinic experience with a focus on patient care and rehabilitation techniques.
- Strong background in acute care or inpatient hospital environments contributing to a comprehensive understanding of complex medical cases.
- Knowledge of physiology, anatomy, medical terminology, and treatment modalities such as trigger point therapy is essential.
- Experience working with geriatric patients or pediatric populations enhances your ability to address diverse needs effectively.
- Familiarity with outpatient rehabilitation center operations and outpatient experience strengthens your capacity for client communication and treatment plan development.
- Proficiency in managing electronic health records (EHR) or EMR systems ensures accurate documentation aligned with clinical standards.
