Pay Rate: $42–$45 per hour
Work Schedule: Monday/Wednesday/Fridays- Part time hours 24 hours a week

Responsibilities of an RN Case Manager:
- No OASIS work!
- Provides care as defined within the scope of the state’s nurse practice act.
- Skilled Cares include, but not limited to: Wound Cares, INR, Catheter Cares, Medication Management, and more.
- Makes the initial evaluation visit and regularly reevaluates the client’s nursing needs.
- Prepares clinical and progress notes and summaries of care.
- Provided On-Call Coverage in rotation with other RNs on team (Additional $200/week pay)
- Assures proper maintenance of clinical records in compliance with local, state, and federal laws.
- Helps educate and coach care-giving staff and assist with new hire clinical training, as needed.
RN Case Manager Requirements:
- Current unrestricted RN license in Minnesota
- Minimum two (2) years of clinical experience with one (1) year of experience in a home care agency, primary care clinic, or health facility, preferred.
- Active CPR Certification
- Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days)
- Reliable car & Auto Insurance
- Valid Driver's License
- Clean background and Criminal Record
- Understands regulatory compliance with state and federal regulations.
- Ability to work on call rotation with compensation
