We are actively hiring Licensed Practical Nurses (LPNs) for day, night, and weekend 12-hour shifts.

What You’ll Do
- Provide one-on-one skilled nursing care in the home
- Perform PDN skills including G-tube care, trach care, oxygen therapy, and suctioning
- Monitor patient condition and report changes to the care team
- Follow individualized care plans to ensure patient safety
- Assist with daily routines and ADLs (including school prep when needed)
- Complete accurate and timely documentation

- 24/7 Coverage Available (Days, Nights & Weekends)
- Full-Time, Part-Time & PRN Roles
- 12-hour shifts available
What We’re Looking For
- Strong clinical nursing experience preferred
- Experience in Home Health, Skilled Nursing Facility (SNF), Assisted Living Facility (ALF), ER, Med-Surg, Rehabilitation, or Long-Term Care settings preferred
- Ability to work independently and provide quality patient care in a home setting
- Strong communication, documentation, and time management skills
- Current LPN license in good standing
- Reliable transportation and valid driver’s license preferred
- Home care or private duty experience is a plus, but not required
