Job Summary

We are seeking a compassionate and dedicated Social Worker to join our healthcare team. The ideal candidate will provide comprehensive hospice social work services to diverse patient populations across various settings, including hospitals, facilities, and in the home. This role involves supporting patients and their families through emotional, psychological, and practical challenges while ensuring adherence to social work practice standards and healthcare regulations. The Social Worker will collaborate with the interdisciplinary team to facilitate patient-centered care, discharge planning, and resource coordination for the end of life.

• Conduct thorough biopsychosocial assessments to identify psychosocial needs and develop individualized care plans. Review and update every 60 or 90 days.
• Provide counseling services including pre-bereavement and anticipatory grief. Supporting the bereaved at visitations or memorial services.
• Coordinate resources such as SNF placement, funeral home planning, waiver programs, housing, financial assistance, and support groups.
• Educate individuals on Code Status and the Georgia POLST.
• Assist with discharge planning by evaluating patient needs and facilitating appropriate placements.
• Maintain accurate medical documentation in compliance with HIPAA and clinical ethical guidelines. Our documentation is to be completed in less than 2 days. Daily report summaries are submitted daily.
• Manage a case load by visiting patients in homes or facilities, updating IDT reviews, and care plans.
• Participate in team meetings for clinical interprofessional collaboration which may be face to face or virtual.
• Develop and lead caregiver or support groups as requested.
• Conduct face-to-face visits to patients, families, and facilities in assigned area.
• Willingness to assist social work team with coverage as needed to ensure visits are completed according to the care plan.

Skills/Experience

• One year of hospice experience required.
• CSWE-approved Master of Social Work degree at time of application.
• Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds. This includes completing FMLA documents, contacting the American Red Cross, submitting referrals to community partners, following up on requests in less than 48 hours,
• Proficiency in crisis management techniques and motivational interviewing strategies.
• Familiarity with medical terminology related to acute care, post-acute care, inpatient settings, and disease trajectories.
• Knowledge of HIPAA compliance and clinical ethical guidelines essential for maintaining confidentiality and professional integrity.
• Competence in researching resources available to patients and caregivers.
• Understanding of mandated reporter duties.
