Develop and maintain strong relationships with clients, their families, and healthcare professionals to ensure seamless care coordination
Demonstrate compassion, empathy, and understanding in all interactions with clients and their loved ones
Maintain accurate records of client care services, including notes, progress reports, and incident reports
Collaborate with the care team to develop and implement individualized care plans that meet clients' unique needs and goals
Participate in ongoing education and training to stay current with industry best practices and regulations
Requirements:
Caregiver experience required, 1 year
Ability to lift, transfer, and move clients as needed
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Reliable transportation and a valid driver's license (if required for the position)
Ability to adapt to changing circumstances and priorities
