Overview
We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will provide comprehensive patient-centered care across various settings, including inpatient, outpatient, skilled nursing facilities, home health, and schools. This role involves developing and implementing individualized care plans to improve patients' functional abilities and quality of life. The Occupational Therapist will work with diverse populations, including children, adults, and seniors, with a focus on enhancing independence through therapeutic interventions. Candidates should possess strong knowledge of physiology, anatomy, medical terminology, and experience working with individuals with disabilities or special needs.
Responsibilities
- Assess patients' functional status and develop personalized treatment plans based on individual needs and goals.
- Assist patients with Activities of Daily Living (ADLs) to promote independence and safety.
- Implement therapeutic interventions for diverse populations such as pediatrics, individuals with autism, or those recovering from acute care or ICU stays.
- Coordinate discharge planning and collaborate with case management teams to ensure seamless patient transitions across care settings.
- Work closely with patients in various environments including inpatient units, outpatient clinics, skilled nursing facilities, home health visits, and school-based programs.
- Provide education to patients and caregivers regarding therapy techniques, adaptive equipment, and safety measures.
- Document patient progress accurately in compliance with healthcare regulations and facility policies.
- Participate in interdisciplinary team meetings to optimize patient care outcomes.
- Support early intervention programs and occupational health initiatives as needed.
- Engage in ongoing professional development to stay current with best practices in occupational therapy.
Experience
- Proven experience working in acute care settings such as ICU or inpatient units is preferred.
- Experience in outpatient clinics or skilled nursing facilities is highly desirable.
- Prior work with pediatric populations in school or early intervention programs is advantageous.
- Familiarity with working with individuals with autism or disabilities across different age groups is a plus.
- Knowledge of post-acute care processes and home health services enhances the ability to deliver comprehensive care.
- Strong understanding of physiology, anatomy, medical terminology, and case management principles is essential for effective practice.
- Previous experience assisting with ADLs, discharge planning, and developing individualized care plans is beneficial.
