- Dementia-Focused Clinical Model – Provide proactive care to patients across all stages of cognitive decline
- Clinic Telehealth Model – Begin with clinic-based care and telehealth, with future opportunities to expand into assisted living and memory care settings
- Cognitive Assessment Support – Collaborate with partnered medical groups to conduct and interpret annual cognitive screenings
- Compensation Range: Annual base salary range of $145,000-$180,000
- Average base salary: approximately $160,000
- Variable incentive plan: based on patient impact, care quality, and clinical outcomes
- Actual pay: determined by experience, skills, and other job-related factors

- Conduct and support annual cognitive assessments in partnership with medical groups
- Deliver dementia-focused care in both in-person clinic and telehealth settings
- Manage chronic and behavioral health conditions related to cognitive decline
- Educate patients and families on disease progression, care planning, and symptom management
- Collaborate across disciplines to ensure coordinated care and high patient satisfaction
- Support growth initiatives in assisted living and memory care communities
- A licensed Nurse Practitioner (NP) or Physician Assistant (PA) in good standing
- Passionate about memory care, geriatrics, and care innovation
- Experienced (or highly interested) in dementia or cognitive health assessments
- Adaptable across clinical settings: in-person, telehealth, and mobile care
- A strong communicator with a compassionate, collaborative approach
