Role Description
This is a flexible, field-based role for a Community Health Worker. The Community Health Worker will engage directly with patients to support their healthcare needs, assist with navigating care plans, provide education related to chronic diseases and treatment adherence, and address social determinants of health. Additional responsibilities include supporting care coordination efforts between patients and healthcare providers and documenting interactions in accordance with organizational workflows and protocols.
