Position Summary

This full-time Preparation Peer Mentor position is part of the Preparation Housing & Services program, that provides short-term housing for 10-12 male-identifying individuals who have completed withdrawal management services and would benefit from ongoing support to enhance long-term wellness goals and community integration.This position is gender responsive, serving male-identifying participants with engagement in ongoing support services, including connection to long-term housing, peer support, employment, and other life-skills and community services. Working in close partnership with the Program Manager, internal and external community partners, the Preparation Peer Mentor acts as a peer role model, documents participant progress toward goals and works collaboratively with participants to increase independent engagement in activities of daily living and improve long-term wellness outcomes.

Essential Job Duties and Responsibilities

•    Serves a diverse population effectively, through management of an assigned caseload of participants while acting as a peer role modeling recovery, and engaging community members in the services of their choice.
•    Understands that participants from various cultural backgrounds experience a variety of needs and can support and assist participants with accessing services that are most important to them.
•    Assists participants in developing a support network by introducing them to various models of recovery in the community and attending community support activities with them.  
•    Assesses needs of participants, connects them with applicable resources and develops a transition plan.  
•    Transports participants and supports them in accessing services of their choice in the location they prefer.
•    Assists participants in securing housing and employment, as needed.
•    Works collaboratively with community partners in a professional manner, including regular interaction with participant’s counselors, medical providers, family members, and other important individuals determined by the participant.
•    Facilitates classes and skill groups, as assigned.
•    Maintains accurate and timely reports and documentation. 
•    Abides by organization policies and procedures, confidentiality laws, professional boundaries, peer support principles and relevant federal/state regulations and standards. 
•    Participates in weekly supervision and meetings, as assigned. 
•    Performs random and as-needed urine drug screenings.
•    Prepares meals when assigned. 
 

Experience, Knowledge, Skills and Abilities
The following are the minimum levels required to successfully perform the Essential Job Duties and Responsibilities.

•    Certified in an Oregon State Mental Health and Addictions approved training as a Mental Health and Addiction Peer Support Specialist or Peer Wellness Specialist, within 90 days of hire
•    Lived experience related to the population served; 2 years of verifiable recovery experience, required
•    Minimum 1 year of continuous sobriety, required and maintained
•    Personal vehicle, valid driver’s license and acceptable driving record in accordance with Bridges to Change’s auto liability policy requirements
•    Food Handlers card, within 30 days of hire. 
•    Demonstrated success at working effectively in a diverse, collaborative team environment, with skills in promoting inclusion and cultural responsiveness
•    Basic understanding of under-served populations within the tri-county area, and ability to offer/access appropriate resources.
•    Excellent communication, trauma informed approach and conflict resolution skills
•    Experience with crisis prevention, intervention and response
•    Experience with de-escalation techniques and ability to manage high risk population effectively
•    Ability to maintain high professional standards at all times with participant, staff, and community partners
•    Computer literate; functional ability with Microsoft programs.
•    GED/High School Diploma, preferred

Working Conditions

Environment and Physical Requirements: The Preparation Peer Mentor works in a housing office and community setting.  This position interacts constantly with staff, visitors, program participants, government agencies, and others. The position stands, walks, bends, lifts, and moves intermittently during working hours. The incumbent must be able to lift, push, pull and move files and equipment in excess of 15 pounds. This position may also be exposed to blood borne pathogens and/or bio- hazardous waste material, as such, adequate training to prevent or limit exposure will be provided.

Position Type/Expected Hours of Work: This is a full-time position. Days and hours of work are dependent on business needs and typically include forty hours per week, 5 days per week. The Preparation Peer Mentor also travels throughout the day in the tri-county area and may be required to occasionally work extended hours including evening, weekend and/or holiday work as job duties demand.
