What You Will Do

As an RN Case Manager, you’ll provide compassionate, skilled hospice care, including:

• Performing physical and psychosocial assessments

• Identifying hospice diagnoses and documenting eligibility

• Delivering skilled nursing care

• Managing pain and symptoms

• Ensuring comfort and quality of life

• Educating patients and families about end-of-life processes

• Developing and coordinating individualized care plans

• Providing emotional and palliative support

• Supervising hospice aides per Medicare requirements

What We’re Looking For

• Active PA Registered Nurse (RN) license in good standing 

• Hospice experience strongly preferred.

• Strong communication and critical-thinking skills

• Willingness to travel within Washington/Fayette Counties

• Valid PA driver’s license, personal vehicle, and insurance

• Ability to lift/move 50 lbs and transfer patient weight of 100 lbs

• Ability to perform physical job functions (walking, bending, standing, etc.)
